Below is a list of billing codes that can be used with Sway Evaluation and Testing.

| 96132 | Neuropsychological testing evaluation services by physician or other qualified healthcare professional, including integration of patient data, interpretation of standardized test results and clinical data, clinical decision making, treatment planning and report and interactive feedback to the patient, family member(s) or caregiver(s), when performed; first hour |

| 96136 | Psychological or neuropsychological test administration and scoring by physician or other qualified healthcare professional, two or more tests, any method, first 30 minutes |

| 96138 | Psychological or neuropsychological test administration and scoring by technician, two or more tests, any method; first 30 minutes. Note specifically that this code requires that at least 2 tests are administered and that they may be administered using any method. This could be paper, verbal, or electronic. Each additional 30 minutes would be billed using CPT 96139. |

| 97530 | requires “use of dynamic activities to improve functional performance. Examples of such activities include lifting, pushing, pulling, reaching, throwing, etc.” Billing this code also requires direct, one on one contact, billed in 15-minute increments. |
| 97750 | Under Physical Medicine and Rehabilitation Tests and Measurements, can be used for a functional capacity evaluation. CPT code 97750 is focused on patient performance of a specific activity or group of activities. |
